The City Clerk serves as the local filing officer for the Fair Political Practices Commission. These reports include Conflict of Interest Statements, Campaign Finance Reports, and Agency Reporting.

Form 802

In 2012, the Fair Political Practices Commission (FPPC) launched an online service providing access to FPPC Form 802 disclosures on the FPPC website.  Please use the following link to access Form 802 disclosure forms for years 2012 - 2015: FPPC website.  As of April 2015, FPPC Form 802 will again be available using the links provided below.

Form 806

The Form 806 is used to report additional compensation that officials receive when they are appointed to positions on committees, boards, or commissions of another public agency or to a committee or position of the agency of which the public official is a member.

  • Form 410 - Statement of Organization Recipient Committee 
  • Form 460 - Recipient Committee Campaign Statement
  • Form 470 - Officeholder and Candidate Campaign Statement 
  • Form 497 - Contribution Report
  • Form 501 - Candidate Intention Statement 

Campaign Finance statements are required of candidates, City Council members, political action committees, and individuals making or receiving $2,000 or more to support or oppose candidates or measures.

All forms are available through the FPPC Website. Most forms may be completed on line and printed out for signature and filing.
